Prepare for Your Care When the Time Comes

Collaborative workshop with Carol Havens, (she is the author of a comprehensive organizer for your loved ones or representative when a medical crisis or death occurs) and Andrea Lantz, certified Respecting Choices ACP Facilitator and Instructor.

This is an educational workshop/presentation where you will be able to watch the Prepare For Your Care film and learn about ways to be proactive in the event of a medical crisis or death.

This event will include a speaker, engaging activities, options to complete documents, and leave with an action plan, including questions to ask their doctors, things to consider, conversation starters, etc.
